পল (এডুওয়ার্ড) ফার্মার মারা গেলেন ২১ ফেব্রুয়ারি, ২০২২-এ মাত্র ৬২ বছর বয়সে (অক্টোবর ২৬, ১৯৫৯ – ফেব্রুয়ারি ২১, ২০২২) সেই সুদূর রাওয়ান্ডায়। তিনি আদতে বস্টনের বাসিন্দা ছিলেন। তাঁর ঘর পাতা ছিল বিশ্ব জুড়ে – হাইতি, পেরু, রাওয়ান্ডা (আফ্রিকা), মেক্সিকো, সিয়েরা লিওন, রাশিয়া, লাইবেরিয়া, লেসোথো, এবং আরও বেশ কিছু দেশে এবং স্থানে।
তাঁর সবচেয়ে পরিচিত গ্রন্থ Pathologies of Power (২০০৩)-এ নির্দ্বিধায় বলেছিলেন – “আমরা যে বিশ্বকে জানি সে বিশ্ব বেশি বেশি করে পরস্পর-সংবদ্ধ (interconnected) হচ্ছে। এর অনুসিদ্ধান্ত হিসেবে বলা যায় যে বিরাট চেহারায় চরম যন্ত্রণা ভোগ করাকে, যেমন গণহত্যার ক্ষেত্রে, কদাচিৎ ক্ষমতাশালীদের ক্রিয়াকলাপ থেকে পৃথক করা যায়।” (পৃঃ ৪২) এ পুস্তকেই পরে বলেছেন – “we must understand that what happens to poor people is never divorced from the actions of the powerful. Certainly, people who define themselves as poor may control their own destinies to some extent. But control of lives is related to control of land, systems of production, and the formal political and legal structures in which lives are enmeshed. With time, both wealth and control have become increasingly concentrated in the hands of a few. The opposite trend is desired by those working for social justice.” (পৃঃ ১৫৮).
কিন্তু পল ফার্মার বিপ্লবী ছিলেননা। একজন তীক্ষ্ণ, নিবেদিত, সমাজমনস্ক চিকিৎসক ছিলেন। তিনি স্বাস্থ্যকে মানবাধিকার হিসেবে প্রতিষ্ঠা করার জন্য লড়াই করেছেন। পৃথিবীর দূরতম প্রান্তের দরিদ্রতম মানুষটির কাছেও কেন সর্বোচ্চমানের চিকিৎসা পৌঁছে দেওয়া যাবেনা? এ প্রশ্ন তিনি বারংবার করেছেন। জনস্বাস্থ্য এবং মূলধারার মেডিসিন চিরে চিরে দেখিয়েছেন এদের মধ্যেকার অন্তর্নিহিত দুর্বলতা কোথায়? শিক্ষিত-অশিক্ষিত, চিকিৎসক-অচিকিৎসক, নীতিপ্রণেতা এবং রাষ্ট্রনেতাদের কাছে বারংবার বিভিন্ন দৃষ্টিকোণ এবং একাধিক প্রাণঘাতী রোগের হাতেকলমে চিকিৎসা করে দেখিয়ে দিয়েছেন কিভাবে অসম্ভবকে সম্ভব করে তোলা যায় – শুধুমাত্র প্রবল ইচ্ছাশক্তি মানুষের মাঝে সঞ্চারিত করে। এবং সেখানে তিনি থেকেছেন জীবন্ত অনুপ্রেরণা হিসেবে।
বেড়ে ওঠার দিনগুলি
ম্যাসাচুসেটসে জন্ম হলেও বেড়ে উঠেছেন ফ্লোরিডাতে – কখনো একটি পুরনো স্কুল বাসের মধ্যে, কখনো নৌকোয়, কখনো টেন্টের ভেতরে। খুব স্বচ্ছল এবং সুখের ছিলনা সেদিনগুলো। কিন্তু লেখাপড়ার আবহাওয়া ছিল বাড়িতে, মানে ভাড়া করা স্কুল বাস এবং টেন্টের মধ্যে।
তাঁর বেড়ে ওঠার সময়ে বিশেষভাবে প্রভাবিত করেছিল জনস্বাস্থ্যের আরেক ক্রান্তদর্শী, দিকপাল চিকিৎসক এবং “সেল থিওরি”র প্রবক্তা রুডলফ ভির্শর লেখাপত্র। ভির্শর বিখ্যাত উক্তি/পর্যবেক্ষণ অনেক চিকিৎসকই জানেন – “Medicine is a social science and politics is nothing else but medicine on a large scale. Medicine as a social science, as the science of human beings, has the obligation to point out problems and to attempt their theoretical solution; the politician, the practical anthropologist, must find the means for their actual solution.” সম্ভবত এরকম এক বোধ থেকেই ডিউক ইউনিভার্সিটিতে তাঁর গ্র্যাজুয়েশন এবং হার্ভার্ডে পোস্টগ্র্যাজুয়েশন করেছিলেন মেডিক্যাল অ্যানথ্রপলজিতে। তিনি মেডিসিনেও এমডি ডিগ্রি অর্জন করেছিলেন। হার্ভার্ড ইউনিভার্সিটির মুষ্টিমেয় কয়েকজনের একজন “ইউনিভার্সিটি প্রোফেসর” ছিলেন। ভির্শ ছাড়াও আরও দুটি ঘটনা তাঁর জীবনকে সম্ভবত প্রভাবিত করেছিল। প্রথমটি, তাঁর সময়কালে মধ্য আমেরিকায় যে মুক্তিসংগ্রাম চলছিল বিদেশী শাসন এবং দারিদ্র্য থেকে মুক্তির জন্য সেসব সংগ্রামের অভিঘাত। দ্বিতীয়টি, সেসময়ে খ্রিষ্টীয় “লিবারেশন থিওলজি”র উত্থান – এই দর্শন দরিদ্রের পাশে থাকার এবং বিদেশি শাসন থেকে মুক্তির কথা শেখায়।
ডিউক ইউনাভার্সিটিতে গ্র্যাজুয়েশনে full scholarship পেয়ে ভর্তি হন। পাস করেন মেডিক্যাল গ্র্যাজুয়েট হিসেবে ১৯৮২ সালে summa cum laude (“with highest praise”) পেয়ে। ক্লাসের ওপরের ১ থেকে ২% ছাত্র এরকম সম্মান অর্জন করে।
১৯৯০ সালে হার্ভার্ড ইউনিভার্সিটি থেকে একইসাথে মেডিসিন এবং মেডিক্যাল অ্যান্থ্রোপলজিতে এমডি এবং পিএইচডি করেন। এই দুটো বিষয়ে একসাথে পড়ার সুযোগ কেবলমাত্র হার্ভার্ডেই ছিল। প্রসঙ্গত বলা দরকার, তাঁর এই কর্মব্যস্ত জীবনে সবার স্বাস্থ্যের জন্য কণ্ঠস্বর তোলা এবং স্বাস্থ্যকে মানবাধিকার হিসেবে পৃথিবীর বিভিন্ন দেশে হাতেকলমে মানুষের মাঝে থেকে প্রতিষ্ঠা করার পাশাপাশি ১২টি পুস্তক এবং ২০০-র বেশি গবেষণাপত্র লিখেছেন – নিউ ইংল্যান্ড জার্নাল অফ মেডিসিন, ল্যান্সেট, ব্রিটিশ মেডিক্যাল জার্নাল, প্লস, আমেরিকান জার্নাল অফ পাবলিক হেলথ, বুলেটিন অফ দ্য হু-র মতো মান্য জার্নালগুলোতে। তাঁকে নিয়ে জীবনীগ্রন্থ লিখেছেন পুলিৎজার প্রাইজজয়ী লেখক ট্রেসি কিডের (Tracy Kidder) – Mountains Beyond Mountainsঃ The Quest of Dr. Paul Farmer, A Man Who Would Cure the World (2003)। যদিও নিজের জীবনীগ্রন্থ লেখার অনুমতি দিতে দ্বিধাগ্রস্ত ছিলেন ফার্মার, বেশ কিছুদিন সময় নিয়েছিলেন।
ছাত্রাবস্থায় নর্থ ক্যারোলাইনার তামাক চাষের ক্ষেতে হাইতির মাইগ্র্যান্ট শ্রমিকদের অবর্ণনীয় অমানুষিক দুরবস্থা দেখে ভেতরে এক প্রতিজ্ঞা জন্ম নিতে শুরু করে। ডিউক বিশ্ববিদ্যালয় থেকে গ্র্যাজুয়েশনের পরে মায়ামিতে Krome Detention Centre দেখতে যান। দেখেন আমেরিকার ইমিগ্রেশন পলিসির মধ্যেকার জাতিগত এবং কাঠামোগত বিভাজন – হাইতি থেকে আগতদের ফেরত পাঠিয়ে দেওয়া হচ্ছে দেশে, অথচ কিউবা থেকে আগতদের আমেরিকাতেই রেখে দেওয়া হচ্ছে।
১৯৮৩ সালে হাইতির কাঙ্গেতে কমিউনিটি-নির্ভর স্বাস্থ্যের কাঠামো তৈরি করেন। ১৯৮৭ সালে আরও কয়েকজনের সাথে মিলে তৈরি করেন Partner in Health সংগঠন। হাইতির অভিজ্ঞতা তাঁর থিসিস (এবং পরবর্তীতে বই হিসেবে প্রকাশিত) Aids and Accusation (১৯৯২) লেখার উপাদান সরবরাহ করে। ১৯৯৪ সালে পেরুতে হাইতির মডেলে সবার নাগালে যাতে স্বাস্থ্য পৌঁছয় এই লক্ষ্যে কাজ শুরু করেন। এমনকি এবোলার মতো মারাত্মক মহামারির সময়ে আফ্রিকার আক্রান্ত দেশগুলোতে পৌঁছে গেছেন তিনি। বারংবার জোর দিয়েছেন ওষুধের ওপরে। বহুজাতিক কোম্পানির ফেরি করা, ক্ষমতাশালী রাষ্ট্রের পলিসি মেকারদের তৈরি করা কস্ট-এফেক্টিভনেসের ধারণাকে নাকচ করেছেন। এ ধারণা যে প্রকৃতপক্ষে দরিদ্রদের কাছে চিকিৎসা না-পৌঁছুনোর অজুহাত মাত্র একথাও তথ্য সহ প্রতিষ্ঠা করেছেন।
মাল্টিড্রাগরেজিস্ট্যান্ট টিবি (MDR TB)-র ক্ষেত্রে তিনি হতদরিদ্র মানুষের মাঝে কাজ করে প্রত্যক্ষভাবে দেখিয়েছেন – (১) নিতান্ত অল্প দামে এ রোগের চিকিৎসা করা সম্ভব, (২) যেকোন সম্পদহীন দেশেই এটা করা সম্ভব, যদি সদিচ্ছা এবং ভিশন থাকে, এবং (৩) দরিদ্রদের শুধু ওষুধ দিলে হবেনা, সাথে খাদ্য এবং পুষ্টির ব্যবস্থা করতে হবে। Ethnography জার্নালে “The Consumption of the Poor: Tuberculosis in the 21st Century” (December 1, 2000) গবেষণাপত্রে তিনি লিখেছিলেন – “In an era in which we have effective therapies, why has tuberculosis remained the leading infectious cause of young adult deaths in much of the world?… in order to answer such a question, we must explore not only the life experiences of those sick with tuberculosis, but also the larger social contexts in which they become infected, fall ill, and meet with a series of therapeutic misadventures leading to complications, ongoing transmission to others and, often enough, death. The `ethnographically visible’ is only part of the story, since the experiences and commentaries of the sick and their providers must be embedded in broader analyses informed by history, political economy, epidemiology and a sociology of knowledge. Such an analysis brings into relief not only cultural specificity but also jarring similarities: living with both poverty and tuberculosis means poor outcomes whether you live in rural Haiti, urban Peru or the inner-city United States.”
২০০৩ সালে বুলেটিন অফ দ্য ওয়ার্ল্ড হেল্থ অর্গানাইজেশন-এ প্রকাশিত তাঁর একটি প্রবন্ধে (“AIDS as a global emergency”, নভেম্বর ২৫, ২০০৩) তিনি দেখান যে বিভিন্ন ক্ষমতাশালী রাষ্ট্র এবং নীতি-নির্ধারকেরা যে কথা বলে থাকেন – এইডসের ক্ষেত্রে চিকিৎসার চেয়ে জোর দেওয়া উচিত প্রিভেনশনের ক্ষেত্রে – তা সর্বৈব ভুল। এর পেছনে লুকিয়ে থাকা একাধিক কারণ তিনি চিহ্নিত করেছিলেন – (১) “There has been scientific progress, but with few dividends for people living with poverty as well as HIV. In most of sub-Saharan Africa, they have access to neither prevention nor treatment.”, (২) “We see claims, for instance, published in leading medical journals, that HIV prevention is “28 times more effective than care.” How is it possible to attain such curiously specific numerical ratios when the cost of care is changing so rapidly? In our Haiti project, ARV prices dropped 90% during the year in which that claim was made … Public health experts should explain to decision-makers that they have to allocate resources in proportion to the problem.”, এবং (৩) “Improving AIDS care has also improved prenatal care and women’s health, tuberculosis case detection and care, and the diagnosis and treatment of sexually transmitted diseases other than AIDS. Equally importantly, improved AIDS care has helped to strengthen long-neglected public health structures.”
তাঁর সর্বশেষ সিদ্ধান্ত ছিল – “ভবিষ্যতের জন্য নতুন দিশা ইতিমধ্যেই স্পষ্ট হয়েছে যদি আমরা ব্যর্থ নীতিগুলোকে পরিত্যাগ করতে প্রস্তুত থাকি।”
এরপরে ২০০৪ সালে ল্যান্সেট-এ প্রকাশিত হয় তাঁর আরেকটি গুরুত্বপূর্ণ প্রবন্ধ “গ্লোবাল হেলথ ইকুয়িটি” (মে ২৯, ২০০৪)। এ প্রবন্ধে তিনি বলেন – প্রতিটি দশকে মেডিক্যাল এডুকেশনের দুনিয়ায় বড়োসড়ো অদলবদল হয়। কিন্তু যে বিষয়টিতে মেডিক্যাল শিক্ষকেরা অপরিচিত কিংবা অস্বাচ্ছন্দ্য বোধ করেন তাহল “health as a human right and the growing disparities of outcome between well-to-do and poor patients.” এপ্রসঙ্গে তিনি মেধার পরিযানের (ব্রেন ড্রেইন) বিষয়টি পাঠকদের নিজরে আনেন – “brain drain draws culturally and linguistically competent clinicians away from their home countries.” তিনি উদাহরণ হিসেবে বলেন যে ১৯৯০-এর দশকে সমগ্র হাইতিতে যতসংখ্যক সাইকিয়াট্রিস্ট ছিল তার চেয়ে বেশি হাইতি-জাত সাইকিয়াট্রিস্ট ছিল কানাডার মন্ট্রিয়লে।
বিখ্যাত অর্থনীতিবিদ জেমস গলব্রেথ তাঁর “A Perfect Crime: Inequality in the Age of Globalization” (Daedalus , Winter, 2002, Vol. 131, No. 1, pp. 11-25) প্রবন্ধে দেখিয়েছিলেন, কিভাবে ১৯৮০-র দশকের পর থেকে নিওলিবারাল অর্থনীতি পৃথিবীতে বিপুল পরিমাণে অর্থনৈতিক অসাম্য এবং অসহায়তা বাড়িয়ে তুলেছে। সে প্রবন্ধে গলব্রেথ জানিয়েছিলেন – “It has been, it would appear, a perfect crime. And while statistical forensics can play a small role in pointing this out, no mechanism to reverse the policy exists, still less any that might repair the damage.”
সম্ভবত গলব্রেথের ধারণার অভিঘাত পড়েছিল তাঁর ওপরে। পূর্বোক্ত লেখায় তিনি বলেন – “মেডিসিন প্রচুর এভিডেন্স তৈরি করছে। কিন্তু ইক্যুইটির কোন পরিকল্পনা এর মাঝে নেইঃ we lack a rights-based approach to its distribution. Medicine and public health goods are still parochial, limited to a few beneficiaries.” বলেন – “যদি স্বাস্থ্যকে কখনও মানবাধিকার হিসেবে নির্মাণ করা হয় তাহলে এই অসাম্যগুলোকে মেডিক্যাল এডুকেশনের ক্ষেত্রে প্রধান চ্যালেঞ্জ হিসেবে দেখতে হবে।” খোলাখুলি বললেন – “Too much conventional international health education shrinks from acknowledging the social roots of grotesque inequalities … too many of us are slow to incorporate rights into our health and teaching practices.” সবশেষে বললেন বিশ্বের নীচের তলার ১০০ কোটি মানুষ “continue to die from readily preventable or treatable disease”। এর উত্তর আমাদের মেডিক্যাল শিক্ষাক্রম দেবে কি?মেডিক্যাল কলেজের শিক্ষাদান নিয়ে এরকম তীক্ষ্ণ প্রশ্ন কতজন আন্তর্জাতিক ক্ষেত্রে এভাবে করেছেন? সম্ভবত একজনও নন। তিনি নিজে PIH (পার্টনার্স ইন হেলথ)-এর কর্মীদের নিয়ে ২০১০ এবং ২০২১ সালে হাইতিতে তীব্র ভূমিকম্পের পরে ঠেলাগাড়িকে ব্যবহার করেছেন স্ট্রেচার হিসেবে, কার্ডবোর্ডকে ব্যবহার করেছেন স্প্লিন্ট হিসেবে। কিন্তু চিকিৎসা থেমে থাকেনি।
আমেরিকান জার্নাল অফ পাবলিক হেলথ-এ তিনি একটি প্রবন্ধ লিখেছিলেন “প্যাথলজিস অফ পাওয়ারঃ রিথিঙ্কিং হেলথ অ্যান্ড হিউম্যান রাইটস” (অক্টোবর, ১৯৯৯) শিরোনামে। সেখানে একজন সত্যদ্রষ্টার মতো বললেন – “Public health and access to medical care are social and economic rights; they are every bit as critical as civil rights. One of the great ironies of the global era, in which public health has increasingly sacrificed equity for efficacy, has been the rejection by the poor of separate standards of care. In our professional journals, these subaltern voices have been heard most clearly with regard to access to antiviral therapy for HIV disease, but the destitute sick are increasingly clear on one point: promoting social and economic rights is the key goal for health and human rights in the 21st century.”
ল্যান্সেট-এ প্রকাশিত (জুলাই ৮, ২০১৩) তাঁর লেখা গবেষণাপত্র “Clinical trials and global health equity”-তে তিনি তথ্যসহ দেখিয়েছিলেন ক্লিনিকাল ট্রায়ালের জন্য যে লক্ষ লক্ষ ডলার খরচ হয় বলে দাবী করা হয় এবং যে দাবীর ভিত্তিতে বলা হয় সবার কাছে একইরকমের স্বাস্থ্যের সুযোগ পৌঁছনো সম্ভব নয় – এ দাবী সম্পূর্ণত ভুল। তাঁর বক্তব্য – “Our own colleagues used rigorous observational methods to reach the same conclusions in Rwanda. The study cost well under US$30000.”
তিনি একদিকে যেমন বড়ো বাঁধ তৈরির ফলে বাস্তুচ্যুত “water refugee”দের স্বাস্থ্যের বিষয় নিয়ে কথা বলেছেন, তেমনি অন্যদিকে হাইতিতে “গণতন্ত্র” প্রতিষ্ঠার জন্য আমেরিকান মিলিটারি যখন হাসপাতাল দখল করে নেয় সে ব্যাপারেও স্পষ্ট মতামত রেখেছেন – “But good news rarely lasts long in Haiti: in early March, Haiti’s newest medical school was turned into a military base for U.S. and other foreign troops. What will become of its faculty, composed in large part of Cuban public health specialists but also including Haitian, U.S., and European teachers?” (“Political Violence and Public Health in Haiti” New England Journal of Medicine, এপ্রিল ৮, ২০০৪)
এক্ষেত্রে তিনি জোর দিয়েছেন “স্ট্রাকচারাল ভায়োলেন্স” বা কাঠামোগত হিংস্রতার ওপরে। তাঁর প্যাথলজিস অফ পাওয়ার গ্রন্থে এর অজস্র বাস্তব উদাহরণ তিনি দিয়েছেন। বলছেন – “But have we gone far beyond AIDS? Is it not a human rights issue that Russian prisoners are exposed, often during illegally prolonged pre-trial detention, to epidemic MDRTB and then denied effective treatment? Is it not a human rights issue that international expert opinion has mistakenly informed Russian prison officials that treatment with second-line drugs is not cost-effective or is just plain unnecessary?” (পৃঃ ২৩৪)
নিউ ইংল্যান্ড জার্নাল অফ মেডিসিন-এ ফার্মারকে নিয়ে একটি প্রবন্ধ প্রকাশিত হয়েছে (মার্চ ৯, ২০২২) “Unclouded Judgment – Global Health and Moral Clarity of Paul Farmer” শিরোনামে। এ প্রবন্ধে লেখিকা ফার্মারকে উদ্ধৃত করে বলেছেন যে তিনি যখন রোগীর সংস্পর্শে আসেন তখন তাঁর বিচারক্ষমতা (Judgment) মেঘমুক্ত (Unclouded) হয়ে যায়। এবং নৈতিকভাবে স্বচ্ছ অবস্থানের জন্য (moral clarity) লক্ষ লক্ষ মানুষের জীবন বেঁচেছে আমেরিকা থেকে হাইতি থেকে পেরু থেকে রাওয়ান্ডা এবং অন্যত্র। এ প্রবন্ধানুযায়ী তিনি দুটি বিষয়কে অগ্রাধিকার দিয়েছিলেন – “স্ট্রাকচারাল ভায়োলেন্স” এবং “সোশ্যাল ডিটারমিন্যান্টস অফ হেলথ”। ফার্মারের জীবনীকার কিডেরকে উদ্ধৃত করে লেখিকা বলেন – “If you say that seven hours is too long to walk for two families of patients, you’re saying that their lives matter less than some others’, and the idea that some lives matter less is the root of all that’s wrong with the world.” যদি তুমি দুজন রোগীর পরিবারকে দেখার জন্য দীর্ঘ পথ হাঁটতে পারবেনা তাহলে ধরে নিতে হবে কিছু জীবনের মূল্য তোমার কাছে কম দামী। এ প্রবন্ধে বলা হয় “among the aspects of academia he disliked were its “arrogance” and “orthodoxy.” বলা হয় – ““modern medicine” should be as much about addressing root causes of poverty and disease as about sophisticated diagnostics and therapeutics.”
অসুখের উৎসকে বোঝা এবং আধুনিক উন্নত রোগনির্ণয় পদ্ধতি ও চিকিৎসাকে সবার কাছে পৌঁছে দেওয়া, এই ছিল তাঁর জীবনের বীজমন্ত্র।
Mountains Beyond Mountains (ফার্মারের জীবনীগ্রন্থ এবং পূর্বোল্লেখিত)-এর লেখক Tracy Kidder এক সাক্ষাৎকারে পল ফার্মারকে নিয়ে বলেছিলেন – “And I was drawn to the man himself. He worked extraordinary hours. In fact, I don’t think he sleeps more than an hour or two most nights. Here was a person who seemed to be practicing more than he preached, who seemed to be living, as nearly as any human being can, without hypocrisy. A challenging person, the kind of person whose example can irritate you by making you feel you’ve never done anything as important, and yet, in his presence, those kinds of feelings tended to vanish. In the past, when I’d imagined a person with credentials like his, I’d imagined someone dour and self-righteous, but he was very friendly and irreverent, and quite funny. He seemed like someone I’d like to know, and I thought that if I did my job well, a reader would feel that way, too.”
এ মানুষটির অমানুষিক কাজকর্ম একজনকে উত্যক্ত (irritate) করতে পারে কিন্তু মানুষটি যে চ্যালেঞ্জিং এ কথা মেনে নিতে হবে – “[H]e was very friendly and irreverent, and quite funny. He seemed like someone I’d like to know, and I thought that if I did my job well, a reader would feel that way, too.”
